求问大佬关于新生上网课TB Health Assessment form的问题

事情是这样的，楼主去年申请到了UC的一所学校，但是因为疫情耽搁了，现在准备春季开始上网课，但是发现注册不了网课，因为UC Immunization hold，于是发信给health service的nurse说明春季是在中国上网课，然后nurse就帮我把immunization hold推迟到了秋季，但是居然还是要求肺结核的检查。所以求问大家 Tuberculosis Health Assessment Form怎么弄，学校要求Download this form and take to your healthcare provider.
